Dr. Sunil Manjila, Dr. Gagandeep Singh & George VijayFounder & President, Founder & COO & CEO & DirectorVis-à-vis the developed western world, in the heavily populated developing nations such as India, Philippines & Vietnam, the ratio between the number of doctors and patients is humungous. As this limits the doctor’s patient visiting time to a bare minimum of five minutes, the documentation of medical history, clinical findings, treatments and future treatment plans becomes a daunting task that is being eschewed by many. It is this clinical negligence that has been the reason behind the sufferings, permanent paralysis or even deaths of several patients nationwide. To illustrate with a simple case, take an elderly person visiting an orthopedician for knee pain in a multi-specialty polyclinic. He might also visit cardiologist & diabeticianon the same day and get medications prescribed from all of them. Some of these medications need supplement tablets, which seem different externally but contains same salts internally and performs similar actions. Taking all of them might become an overdose and can cause serious damage, unless the clinic has an integrated system that alerts the doctors about previous prescriptions.

Having witnessed such tragedies and realizing the paramount importance of a system that documents all clinical obligations, Dr. Sunil Manjila (Founder, President & an illustrious neurosurgeon), Dr. Gagandeep Singh (Founder, COO & a gold medallist MBBS graduate from Punjab University) and George Vijay (Co-Founder, CEO &an innovative technologist who has contributed as an architect for the World’s top two EMRs) incepted Manjila and Singh Innotech (MASIT) in early 2014.After meticulously researching and understanding how Indian doctors check their patients, MASIT created its SaaS-based MedAlert suite with a highly intuitive & analytical interface to bring down the documentation time drastically – a vital attribute many EMRs that are successful in western world have failed to take into account. All the doctors have to do is tick the check boxes in the intuitive documentation drafted like a questionnaire in a couple of minutes, instead of typing entire information like western doctors. This empowers doctors to not just manage drug and maintain the clinical history of patients with chronic diseases like diabetes & hypertension, but for each and every patient through out their lifetime.

MedAlert– Empowering Effective Solutions
MedAlert lays immense emphasis on the clinical aspects of a hospital – particularly on clinical documentation, as opposed to other Hospital Systems that merely focus on the operational & business management aspects. “There is little documentation that needs to be logged in order to give us proper analytical reports and analytical notifications to all these stakeholders – Providers, Payers as well as Patients,” George adds. Serving to three Ps, MASIT makes Patients aware of their condition, equips Providers (physicians, doctors, nurses, lab technicians & hospitals) with best possible treatment for their patients and keeps Payers (insurance companies) informed regarding their customers/patients to best serve them.

Aptly named as MedAlert, this GST-ready product alerts doctors based on several parameters of the patients’ data, including health history, demography, age, gender and so on. Catering EMR products for the hospitals & clinics in Indian, South-East Asian and Middle-East markets, the company wields the power of data analytics to advice doctors on differential diagnostics, possible treatments &medications and the risks a patient can have based on his/her prior histories from both family and social sides – a highly innovative & much yearned feature.

Offered as a solutions suite, MedAlert Health Framework encompasses several modules, namely Electronic Medical Record, Electronic Health Record, Practice Management (appointments, scheduling, billing, accounts& others), Lab Information System(integrating fully automated lab machines with Med Alert),Radiology Information System(integrating the complete digital radiology equipments like digital X-rays, MRIs, ultrasound scanto MedAlert directly) and a robust analytical solution on top of it. MATIS has its own DICOM systems and proprietary drug information system which contains complete drug pharmacopeia for over 1800 generic medications, which gets constantly updated to include all the brand details currently available in India. Understanding that one solution doesn’t fit all, the application’s top layer is built to easily accommodate customizations based on the requirements of every single doctor in a particular hospital.

Yet another reason for Indian medical institutions to shy away from the western EMRs is their exorbitant cost; since unlike the western world where most of the payments are made by insurance companies, individual physician hospital (or medium scale hospitals for that matter) in
developing countries are unable to afford multi-crore automated projects. This is where MedAlert steps in with its SaaS based model, where the hospital management doesn’t burn on any upfront infrastructure investment. No matter how many facilities (doctor visit, pharmacy, investigation & others) have been used by the patient within the hospital, the management only pay one static amount per patient as the processing fee. This model is quite welcomed by the doctor fraternity and the hospital communities, as they can utilize a sophisticated system at a fraction of the cost that can be shown as an operational expenditure (and not CAPEX) which works better on their accounting books. As a cherry on top, clients can buy only the modules they need amongst the MedAlert suite.

Predictive & Reactive Analytics
MASIT’s reactive analysis shows past insights such as previous quarter’s profits, number of patients visited, diseases encountered and average patient interaction time among others, which facilitates the management to improve the processes. To demonstrate, the data regarding which doctor had the longest waiting time would help the management to take a decision about adding another additional body to take care of the patients. Apart from this management related analytics, clinical related analytics gathered based on the patient’s demography, age, gender & others would help the doctors to figure out the best outcome for a particular patient belonging to a specific gender, age & so on,thus provide the most effective treatment. MedAlert also utilizes predictive analytics to forecast the future, which could be as simple as stocking more acetaminophen during monsoon time or as complex as diagnosing a woman attaining menopause with the risk of contracting osteoporosis, based on her medical history and help her take preventive measures such as diet or calcium tablets.

Considering the founders’ western background, unsurprisingly MASIT vests keen importance on security & privacy and hence takes all the measures prescribed by HIPAA and implement it. Likewise, it uses ICD-10 code (international coding for diseases), SNOMED CT for all clinical terminologies and LOINC standard for all labs. The company that recently mitigated its product to bleeding-edge technologies encourages its employees to get trained through self-learning methods (video & internet based learning) for free of cost, while MASIT subscribes for them. It informs employees regarding the product’s future roadmap and sets them training goals to achieve as a part of the appraisal system. Besides the doctors MASIT has on-boarded in India, it also has four doctors in U.S. to assist in developing clinical processes.

New Mountains to Climb
With the investments from Abraham Oommen (VP – Business Development), Dr. Harendarand other small scale investors from Haryana and Kerala, MASIT has achieved a phenomenal growth of 400 percent in revenue, 300 percent in user base and 500 percent in patient acquisition (40,000 to 200,000 patients) from last year. MASIT is projecting to host one million patient acquisition records in its system by March 2018, 10 million in three years and 100 million in five years. Currently in talks with international companies like GE (Dubai) and Case Western University (Ohio), MASIT intends to raise Series A funding next year in order to scale up its operations to multiple countries.

With the intention of obtaining data from patient’s side as well, the company plans to roll out wearable healthcare solutions for patients from next year. Besides launching patient engagement systems akin to mobile apps and NFC cards, MASIT also plans to focus on deploying its analytic products alone, which would integrate with its existing EMRs. Shedding its spotlight on bridging the gap in multi-speciality poly clinics at present, MASIT aims to deploy inpatient, bed management and hospital management systems in hospitals from the next two quarters. It has already tied-up with a 500-bedded hospital in Kerala and is in talks with a Kerala-based 100-bedded teaching institution & a Thanjavur-based hospital. Taking such long strides, the company seems to have the perfect momentum to reach the topmost spot in the industry.

Case Study
Dr. Joseph’s Ortho Clinic, a polyclinic with senior orthopaedic surgeons & cardiologists among others, a full-fledged pharmacy with modern equipments and fully automated labs tried MedAlert as their sixth software, as all the previous five failed to meet their expectations – especially pertaining to the clinical aspects of business. The implementation of MedAlert after the personal analysis of MASIT, however, completely changed their way of doing business.

Besides the tremendous increase in the quality of patient care caused by complete integration (for instance, elimination of medication over-dosage through duplicate prescription alert) and acceleration of documentation process, the pharmac's average sales per month skyrocketed from Rs.5 lakh to Rs.9 lakh, since they eliminated medication theft by keeping a tab on stock frequently with MedAlert’s random stock taking facility. Similarly, the integrated lab with barcodes stopped people from testing the samples brought from outside illegally, as now all the data is being fed back into the system.
