Dr Piyush Goel

Senior Consultant and Critical Care Specialist

Amidst the globally advancing healthcare realm, India has long proven its stance to be one of the greatest healthcare providers leveraging improved medical care and treatment to patients worldwide. Like several other healthcare specializations, pulmonology care is witnessing demands, and today, we have the latest and advanced treatment available for patients in the country, deploying skilled professionals, doctors, and staff. Pulmonology care faces continuous challenges due to acute cases of respiratory illnesses, with an increasing prevalence of respiratory infections presenting patients with both curable and non-curable lung diseases. In this context, it is imperative to acknowledge the exemplary work of dedicated pulmonologists who provide exceptional patient care.

His Educational & Professional Journey
One distinguished expert in this field is Dr Piyush Goel, a specialist in tuberculosis and chest diseases & interventional pulmonologist, who practices in Gurgaon. He holds the position of Senior Consultant and Critical Care Specialist at Narayana Hospital. Dr Goel obtained his MBBS degree from Government Medical College, Patiala, Punjab, and subsequently completed his MD in Tuberculosis and Respiratory Diseases from the same institution. He furthered his education with a fellowship in Critical Care Medicine at Sir Gangaram Hospital, New Delhi, and has been active in the Delhi NCR region since that time.

In terms of his professional experience, Dr Goel has an extensive background, having spent nearly a decade at Manipal Hospital, Gurgaon. He has recently joined Narayana Hospital, where he serves as the Head of the Pulmonary and Critical Care Department. Throughout his career, Dr Goel has also completed many advanced courses and fellowships. Some of those are in interventional pulmonology and medical Thoracoscopy/Pleuroscopy, including some provided by the European Respiratory Society (ERS). He also has a European Diploma in Adult Respiratory Medicine from Spain.

The Demanding Lungs & Respiratory Cases
Today, many individuals suffer from lung diseases. Some of the most common conditions he currently manages include patients with COPD, asthma, tuberculosis (TB), and those needing interventional pulmonology procedures such as medical thoracoscopy/pleuroscopy. He also cares for patients with acute respiratory distress syndrome (ARDS).
Having completed his fellowship in critical care medicine at Sir Gangaram Hospital, he has treated numerous patients during the COVID-19 pandemic too, who required ARDS management and ECMO support.

Additionally, he is dedicated to serving patients with severe pneumonia and other lung diseases. He also cares for patients who experience hemoptysis, where they cough up blood from the lungs. Many patients come to him with breathing difficulties, including geriatric patients who have multiple chronic conditions affecting their lungs and hearts. A few diseases like COPD are non-curable but can be managed through proper care & treatment. On the other hand, some conditions are curable, but accurate diagnosis often requires on minimally invasive interventional procedures and biopsies. However, there are two main challenges in treatment, namely the disease itself and the patient's family, who must ensure positive treatment results, finances, and more. Notably, his focus remains on providing comprehensive care for all patients in pulmonology.

Focus on Technological Advances
As new technologies, particularly AI advance, diagnosing serious diseases like lung cancer will become more accurate and timely. In the future, AI combined with CT scans could potentially predict lung cancer risk based on a person's habits, such as smoking. This could encourage preventative actions, as many smokers currently ignore the risks. Moreover, technologies like endobronchial ultrasound-guided FNAC allow for early detection of small lymph nodes in the lungs that were previously undetectable. This technique helps diagnose conditions like sarcoidosis and lung cancer at earlier stages, enabling better treatment. Innovations, including radial leverage and electromagnetic navigation, facilitate access to difficult-to-reach lung nodules. Overall, these advances, along with new medications and vaccinations, improve patient outcomes, enhance quality of life, and reduce hospitalizations.

Dr Piyush says that his mission is to enhance patients' quality of life while committing to continuous education in new techniques

Preventive Measures
The lungs are exposed to harmful pollutants in the air, which can lead to serious health issues like heart attacks and lung diseases. To protect them, one should wear masks (N95 or surgical), get vaccinated (influenza and pneumococcal), and have regular check-ups, especially if the risk seems to be higher. Also, staying active with exercise and deep breathing, and avoiding smoking, is beneficial as it significantly harms lung health. Prioritizing these practices aids in maintaining long-term lung health.

Future Roadmap
Dr Piyush has had a rewarding career of almost two decades, during which he received recognition from many, including positive patient feedbacks. Patient satisfaction is one of the most notable achievements he believes he has received from his patients.

Furthermore, too, he aspires to increase his practice's patient volume by offering a comprehensive range of interventional procedures and critical care. He says that his mission is to enhance patients' quality of life while committing to continuous education in new techniques. Working at Narayana Hospital, he aims to provide a complete treatment package, minimizing the need for referrals to other institutions.
Dr Piyush states, “A significant challenge in India is that patients often fear treatment more than the diseases themselves, which can lead to poor health management. My objective is to shift this mindset, particularly among the younger generation, to encourage a more proactive approach to their health”.
